Order Form Help

1)	You must fill out the required fields shown:
a.	Contact Name
b.	Company Name
c.	Shipping Address
d.	Contact Phone Number
e.	Your Purchase Order Number
f.	E-mail address is optional
g.	Last 6 digits of the serial number ( Freightliner, Western Star, Sterling)
h.	Choose method of Delivery
i.	Enter Qty's & Part Numbers and/or Descriptions of the parts

2)	 You may type any notes/comments in the text box at the bottom.

3)	Click the "Submit" button to send the form or "Reset Form" to clear the entries
